We would like to highlight some important information you need to know for 2012. Below you will find helpful information and resources to make the transition go smoothly.
Important notices:
- The Health Choice 2000 Plus plan will be available for both stateside and overseas employees. There will be no separate plan for overseas employees.
- You will be automatically enrolled in the new Health Choice 2000 Plus plan effective January 1, 2012.
- You will receive new ID cards for 2012. Note that your personal ID number will not change.
Benefit enhancements for 2012:
- The annual coinsurance maximum for an individual/family will be lowered from $5,000 to $4,000.
- The specialist copay will be lowered from $45 to $35.
- The copay for chiropractic services will be lowered from $45 to $35.
- Qualified vision services received from out-of-network providers will now be covered at 50% after your deductible.
Prescription drug changes for 2012:
- The prescription drug deductible has been eliminated from your plan.
- Retail 30-day prescription copays will change to 20% with a per-prescription maximum of $250.
- Mail order 90-day prescription copays will change to 20% with a per-prescription maximum of $750.
