Complete your Application in Two Steps

To apply for Personal Plans term life, disability or medical coverage, follow the steps below.

  1. Confirm your Personal Plans eligibility.
  2. Complete necessary form(s):
  3. Evidence of Good Health Application
  4. Only Secure Health Plans (3000 or 8000)
  5. All other Personal Plans Coverage*
  6. Employer Certification of Participant Status Form (only if your Evidence of Good Health Application includes medical coverage)
