How to Apply for GuideStone Personal Plans Coverage

Complete your Evidence of Good Health Application in Three Steps


To apply for Personal Plans term life, disability or medical coverage, follow the steps below.

  1. Confirm your eligibility to participate in GuideStone's insurance products and services by reviewing the Personal Plans eligibility and enrollment guidelines.
  2. Submit necessary forms:
  3. Complete an Evidence of Good Health Application when applying for any personal plan coverage option.
  4. Complete an Employer Certification of Participant Status Form if your application includes medical coverage.
  5. Email Insurance@GuideStone.org to request a secure email*, and reply with the above form(s) as attachment(s).

*The Evidence of Good Health Application contains private health information and should not be sent via an unsecured email.

Please allow six to eight weeks for your application to be processed. You will be notified about the enrollment decision in writing.
