Dental Plan Enrollment

To enroll in a dental plan:

  • Review your dental plan options
  • Contacting us via phone or email Insurance@GuideStone.org
  • Complete the Certification of Dependent Eligibility if you want your spouse or your children to be included in your plan.

You may enroll in dental coverage at any time.

Term Life & Accident Plan Enrollment

To enroll in term life and accident plans:

  • Review your term life and accident options
  • Apply Now
  • Complete the Certification of Dependent Eligibility if you want your spouse or your children to be included in your plan.

You may apply for term life and accident plans at any time. Please allow six to eight weeks for your application to be processed.

Disability Plan Enrollment

To enroll in a disability plan:

  • Review your disability plan options.
  • Apply Now.

Medicare-coordinating Plan Enrollment

To enroll in a Medicare-coordinating plan:

  • Review our Care Plans for retirees.
  • Apply online by completing the Personal Plans Medicare-coordinating Plans Packet.

Seminary Student Enrollment

If you are a student at a GuideStone-approved seminary, college or Bible school, you may be eligible for Open Enrollment in the Personals Plans for Seminarians if you meet all of the following guidelines:

  • You must apply during the first 60 days of your initial registration date.
  • You must not have attended or obtained a degree from another GuideStone-approved seminary, college or Bible school.
  • You must attend a school that is eligible to utilize products and services made available by or through GuideStone. Some schools will require enrollment in an official degree program that is intended to qualify you for a full-time position in a ministerial field.

Medical Enrollment

Medical enrollment is only available for paid employees working 20 or more hours per week.

  • If you want your spouse or children in your plan, complete the Certification of Dependent Eligibility.

Dental, Term Life and Accident Plan Enrollment

To get a quote for dental, term life and/or accident plans, contact us or use our online Get A Quote tool.

  • Review your dental plan, term life and accident plan options.
  • Enroll in the plan by completing the Seminarian Open Enrollment Form.

If you are not eligible for open enrollment or if you are requesting term life coverage for your dependents, you must complete an Evidence of Good Health Application for Seminary Students by applying for all family members requesting coverage. Complete the Certification of Dependent Eligibility if you are applying for the Spouse Term Life Plan or the Child Term Life Plan for your dependents.
