Personal Plans Eligibility and Enrollment Guidelines

Review the requirements below to determine eligibility for GuideStone's Personal Plans.


Who is eligible for GuideStone's Personal Plans?

An individual may be eligible to participate in GuideStone's Personal Plans if he or she is an active employee who meets the following requirements:

Eligible Employer

Is earning wages from a church or ministry organization eligible to offer products and services made available through GuideStone®

Minimum Hours Requirement

Works 20 or more hours per week

Waiting Period Completed

Has completed their employer's waiting period, if any

Health Plans

Eligible employees who work for an employer with 10 or fewer paid employees may be considered for coverage by submitting the Personal Plans Evidence of Good Health (EOGH) Application at any time. The application will be required during the following:

  • New Employment: New employees must submit an EOGH to enroll in Personal Plans for coverage at their new employer.
  • Special Enrollment Events: When a current enrollee's family status changes due to one of the following special events — marriage, birth, adoption or placement in the home for adoption — the dependent experiencing the special event may be eligible to enroll in the health plan without an EOGH. All other family members must submit an EOGH application.
  • Enrolling Dependents: Eligible spouses and children, including children under 19, may apply for coverage at any time by completing an EOGH application.

Please note: Employees of employers with more than 10 employees are not eligible to participate in Personal Plans. Employers with more than 10 employees may explore health plan coverage options. Learn more about our Group Plans available to employers with at least two covered employees.

Disability and Term Life Insurance

Eligible employees are required to submit the Personal Plans Evidence of Good Health (EOGH) Application for disability and term life insurance for themselves and all family members requesting term life insurance.

Employees approved for term life insurance are automatically eligible to add Accidental Death and Dismemberment (AD&D) for the same amount as their term life insurance.
