What are the eligibility requirements for GuideStone Personal Plans?

GuideStone Personal Plans are available to employees of evangelical churches, agencies or institutions affiliated or sharing common religious bonds with the Southern Baptist Convention (SBC).

An individual is eligible to participate in GuideStone’s Personal Plans if he or she meets both of the conditions listed below:

  1. Is a paid employee of an eligible church, agency or institution affiliated or sharing common religious bonds with the SBC
  2. Works 20 or more hours per week.

Eligible employees may enroll in a GuideStone Personal health plan without providing an Evidence of Good Health Application if they experience one or more of the special enrollment events listed below. Employees must notify GuideStone within 60 days of the date they experience the event. Coverage will be effective on the date of the special enrollment event.

  • New employment: New employees are eligible to enroll in a plan on their first day of work at an eligible employer.
  • Work schedule increases to 20 or more hours per week: Current employees are eligible to enroll in a plan at an eligible employer if their work hours increase to more than 20 hours per week.
  • Adding a dependent: Current employees who add a dependent to their family through marriage, birth or placement for adoption may add that dependent to their plan. Coverage eligibility begins on the date of the marriage, birth or placement for adoption date. Employees currently enrolled in medical plans may add dependents younger than 19 at any time. For an employee to add a dependent older than 19, the employee must experience a special enrollment event. The dependent will need to provide an Evidence of Good Health Application if the employee works for an employer with 10 or fewer paid employees.
  • Loss of other coverage: In response to the COVID-19 crisis, GuideStone is temporarily suspending the requirement that those experiencing loss of other coverage must submit an Evidence of Good Health Application when applying for Personal Plans coverage. This change in our enrollment procedures will remain in place through April 1, 2021. Initial requests for enrollment for potential members or additional dependents must be received within 60 days of the loss of other coverage event. Proof of loss of other coverage is required within 30 days of initial request and before coverage can begin.

Your timeline for reporting special enrollment events may be extended due to the COVID-19 pandemic. Contact Customer Solutions at 1-844-INS-GUIDE (1-844-467-4843) Monday-Friday, 7 a.m. to 6 p.m. CT.

Individuals who work for an employer with 10 or fewer paid employees may apply for coverage by submitting the Evidence of Good Health Application at any time.

Please note: Employees of employers with more than 10 employees must experience one of the special enrollment events listed above to enroll. These employees may also be eligible to enroll in the employer’s group plan.

What types of health plans are available through GuideStone?

GuideStone offers a variety of health plans designed to match your budget and health care needs. You can choose a comprehensive PPO plan or a consumer-driven plan — such as a high deductible health plan or a more budget-friendly protection plan.
